You must printthe form (PDF) and have a physician, nurse, social worker, rehabilitation counselor or other qualified individual complete the form and return it to WXXI Reachout Radio at P.O. Box 30021, Rochester, NY 14603-3021.

As soon as we receive your completed application and certification of disability, we will contact you about the receiver.  Several different receiver models are available and we will select the option that will provide you with the best reception possible.  There are a limited number of portable receivers that require 4-C cell batteries.  Applications are filled as available.
